Mid-air collision with N4876 5m SW of Brocklesby, NSW, 29.9.40 (Pick-a-back ldg). 1 AD 1.10.40. Converted to Instructional Airframe No 4 19.11.40. Approved to convert to components 20.6.43. ‘Ansons L9162 and N4876 of 2 SFTS collided when N4876 came down on top of L9162 in the air near Brocklesby, NSW, on 29 September 1940. The aircraft remained stuck together. The crew of the lower aircraft (Leading Aircraftsman [LAC] Jack Hewson and LAC Hugh Fraser) and the observer from the upper aircraft (LAC Ian Sinclair) baled out. The pilot of N4876 (LAC Leonard Fuller) managed to fly the two aircraft and belly land them, still joined, safely, in a paddock belonging to Mr T. Murphy approximately 7 miles south west of Brocklesby . L9162 was written off but N4876 was repaired and continued to fly until the end of the war. LAC Fuller went on to fly with the RAAF in Europe and won the DFM. Unfortunately he was killed at East Sale in 1944 when he was hit by a bus while he was riding a bicycle.

Crashed into the Richmond Golf Course almost immediately after take-off from Richmond, NSW, in the early hours of the morning on a flight to Point Cook, Vic, 18.12.39. The aircraft took off at about 1.30 am, seconds before another aircraft was to take-off. Ground crew heard a loud roar then the sound of a crash. Many of the ground crew ran with torches and flares to the area of the sounds and found the aircraft debris scattered over a wide area. One engine had been driven deep into the earth. Flight Lieutenants Arthur Moorhouse Watkins and Hugh B. Horner, Flying Officers Henry Parker Fitzgerald and Malcolm Musgrave McInnes and Leading Aircraftsman Leonard J King all died instantly, some bodies being found a significant distance from the aircraft.

He then came in low over the oval and clipped the tops of the big gums we used to have around the oval, he tried to pull up, it lifted a bit then dove straight into the paddock, we all ran to see if we could help but it burned as soon as it hit the ground, the poor buggers. We only found three bodies that night, we found the other bloke the next day. I said to my mate that the left engine was on fire, but he said it was probably the exhaust, the engines were making a hell of noise, really screaming they were. Nobody could approach the site for several days as the police had cordoned it off, as one farmer said, "Bloody tight security we couldn't nick a thing". Eyewitness Report 2: "We heard a thump that night, next day after school we went to the place, but there was only a burned out wreck and the police had strong security around it." His wife saw the plane approaching very low, it went over the house then hit the ground beyond some trees about 3/4 mile from the house. There was an explosion a few minutes later and a cloud of smoke was seen rising. When Mrs Harvey arrived at the site, the wreck was a flaming mess with machine gun ammunition going off constantly. The sound of the crash was heard for several miles. The bodies of the 4 crew were flung from the plane, the rudder catapulted over the wreck and landed up a large tree about 20 metres further on from where the plane landed. The remains of F/O Birt were interred in the Northam Cemetery (Anglican section) on Sunday October 11th, 1942. From Queensland Air Museum "It should be noted that for many years there was some doubt as to the correct military identity of VH-BIF which was thought to be W2472, but it is now believed to be AX305. Anson AX305 is understood to have a fuselage number of R3/LW/107500. This number is carried on a data plate which was found affixed to the fuselage of VH-BIF at the time of its recovery from Charleville. Anson W2472 is understood to have a fuselage number in the range R3/LW/88521 (W2434) to R3/LW/88569 (W2486). Anson W2472 has since been located at Miles, Qld. As these numbers were not allocated in strict sequence, it is not possible to extrapolate corresponding military serial numbers or vice versa. The weight of evidence now suggests that VH-BIF is ex AX305."
The RAAF Amberley Aviation Heritage Center acquired W2472 in 2010. Although there was initial uncertainty about its ID they are now confident that it is in fact W2472. It was located at Roma having previously been located at Miles.
It is now under restoration at the RAAF Amberley Aviation Heritage Center
